Apply for Temporary Housing

Applying for temporary housing at The Ballard House requires TWO STEPS:

STEP ONE: Submit the application below which covers the patient, medical, caregiver, and housing details we require to process your request for temporary housing. Fill in the information and click SUBMIT at the bottom. If you would prefer to complete the application with pen and paper, click here to download the PDF version of the application, complete the form, and then scan and send the application to us by email to applications@cincocharities.org or fax it to 281-220-6417.

STEP TWO: We must receive a referral submitted by a Houston-area hospital or medical facility, physician, social worker, or community service or outreach organization. You can download the referral here and provide it to your physician or social worker who must complete it and submit it directly to us per the instructions on the form.

We make every effort to accommodate your needs, but please note our housing is provided based on availability in our facility. An application is not considered complete until BOTH steps above have been completed. We will notify you when your application has been approved and we have confirmed housing is available to suit your needs.

Please read the following housing policies and procedures for The Ballard House:

  • OUR HOUSING: Our temporary housing is available for patients and their caregiver / family when the patient has a need to travel to the Houston area for medical treatment for a life-threatening illness. The approved length of stay will be determined by The Ballard House and cannot exceed 90 consecutive days. Subsequent stays require a new application which must be approved by The Ballard House. Lodging at The Ballard House is temporary and may be terminated by us at any time.

  • CHECK-IN: Check-in time will be arranged by appointment: Monday through Friday between 2:00 PM and 7:00 PM, and Saturday and Sunday between 1:00 PM and 5:00 PM. Late check-ins are not permitted.

  • CHECK-OUT: Guests must check-out by 11:00 AM.

  • DEPOSIT: There is no cost to stay at The Ballard House. During check-in, The Ballard House will collect or hold a $100 deposit via credit or debit card to cover incidentals or damage that may arise. The deposit will be returned to you upon checkout, or you may elect to donate all or a portion of the deposit to help fund ongoing operational expenses at The Ballard House. Donations are greatly appreciated, but they are not required.

  • AMENITIES: Clean bedding, towels, and bathroom cleaning products are provided in each guest room. On-site laundry facilities are free for guests to wash clothes and linens during their stay. Guests must provide their own food and toiletries.

  • SAFETY: The Ballard House does not allow any smoking, alcohol, illegal drugs, pets, weapons, or disruptive or harmful behavior. Patients, caregivers, family, and overnight guests agree for a background check to be conducted by The Ballard House.

  • TRANSPORTATION: Guests are responsible for arranging their own transportation to and from The Ballard House and around the Houston area.

  • INDEMNIFICATION: Guests agree to indemnify and hold Cinco Charities, Inc. and The Ballard House, its officers, directors, agents and assigns harmless from any and all liability arising out of or connected with their stay at The Ballard House.
